Hospice provides high quality, compassionate end-of-life care for people with terminal illness. Hospice focuses on caring, not curing and is usually provided in the patient’s home, but can be delivered in Nursing Homes and Hospitals where the Hospice Provider has a contract. Patients on Hospice are entitled to go out as they feel able to do so.
Hospice provides ethical and compassionate medical & nursing care with an emphasis on pain management and symptom relief. Hand in Hand Hospice places value on the preservation of patient dignity as well as comfort and bereavement support for family and loved ones at the end of life cycle.
An interdisciplinary team of health care professionals that includes physicians, skilled nurses, therapists, social workers, chaplains, bereavement specialist, home health aides and trained volunteers provide Hospice care. The care is directed by the patient’s own physician in collaboration with the hospice staff. Alternatively the Hospice Medical Director may oversee the management of the patient and the care delivered by the hospice team of skilled personnel. |
